Summary
We have classified pelvic tumor resections into five types according to the disruption of the structural continuity of the pelvic ring, sacrifice of the hip joint, and the necessity for reconstruction. Type IA is a resection of the iliac wing; type IB is a resection of the ischium; type II is a resection of the anterior part of the pelvic ring; type III is a resection of the periacetabular area. In type IIIA, only the acetabulum is resected; in type HIB, both the acetabulum and the femoral head are reseted. Type IVA is a partial resection of the unilateral sacroiliac joint. Type IVB is a total resection of the unilateral sacroiliac joint. Type VA is a partial resection of the sacrum. Type VB is total sacral resection.
Reconstruction was not necessary in types I and II. Restoration of the hip function by arthrodesis or prosthetic replacement was always necessary in type III. Reconstruction of the pelvic continuity was indispensable in types IVB and VB. The necessity of restoring pelvic stability was dependent on the amount of sacroiliac joint resection in types IVA and VA.
