Zusammenfassung

Die infektiöse Endokarditis hat in den letzten 50 Jahren eine erhebliche Wandlung erfahren (Weinstein 1973, 1980; Eichstädt 1979). Dies betrifft einerScits das Patientengut, in dem in zunehmendem Maße ältere Patienten betroffen werden; so hat von 1930 bis 1960 das mittlere Alter von ca. 30 auf 45—50 Jahre zugenommen (Thayer 1926; Lode 1977; Schnurr 1977) und die infektiöse Endokarditis ist auch bei über 70jährigen keine Seltenheit mehr (Habte-Gabr 1973) (im eigenen Krankengut sind 12% der Fälle über 70 Jahre) mit einem deutlichen Überwiegen der Männer im Verhältnis von ca. 2 zu 1 (Lerner und Weinstein 1966). AndererScits hat sich das Erregerspektrum geändert, indem sich zu den klassischen Erregern wie α-Streptokokken bzw. Streptokokkus viridans weniger übliche wie z.B. Staphylokokkus aureus oder Enterokokken, Pseudomonas, Serratia, aber auch Gram-negative Keime und Pilze, vor allem Candida albicans, dazugesellt haben (Eichstädt 1979; Muegge 1982).
