Abstract

Lung carcinoma is currently perceived to be the result of complex mutational and epigenetic changes in the epithelial cells of the lung that impart to those cells a high capacity for proliferation, resistance to programmed cell (apoptosis), the ability to invade and destroy normal tissue and the capacity to metastasize. Lung cancer cells achieve these properties through series of genomic changes that are highly heterogeneous with respect to the timing of their occurrence, their complexity, the anatomic location of affected cells and the specific molecular pathways that are affected. Defining these changes, devising methods for measuring them in large patient populations and taking advantage of these changes in therapeutic protocols are and will continue to be the most daunting challenges in lung cancer research for the foreseeable future. In recent years, there has been a large increase in available molecular and cellular information in this field. This review attempts to relate well-established molecular data to the clinical problem of lung cancer and premalignancy. Numerous references referring to more detailed reviews are provided for the convenience of those interested in further reading.
