Primary Arthroplasty for Fractures of the Proximal Femur

Abstract

In this chapter, the authors explore the principles, indications, and technical considerations for primary joint arthroplasty in fractures of the proximal femur. In younger patients, the first choice for surgical management of these fractures is to attempt osteosynthesis. In the elderly, however, joint reconstruction could be more challenging due to poor bone stock, osteoporosis, and significant comorbidities. Multiple operations are best avoided in these patients; a primary arthroplasty surgery may be the best approach in order to have a quicker recovery and avoid the risks associated with non-union and avascular necrosis. This chapter reviews the current literature on managing such fractures with either primary arthroplasty using hemiarthroplasty or total hip replacement alone, or in combination with reconstruction/fracture fixation, and choice of available implants.
