Prostatakrebs – Prävalenz, Bedeutung und Implikationen für die Prävention und Gesundheitsförderung

Zusammenfassung

Das Prostatakarzinom (PCa) ist mit 23 % aller Krebsneuerkrankungen der häufigste maligne Tumor (außer Malignome der Haut) des Mannes und mit 11,3 % aller Krebssterbefälle auf Platz zwei der krebsbedingten Mortalität in Deutschland. Das PCa wird durch den demografischen Wandel in Deutschland an Bedeutung gewinnen, da es sich um einen Tumor des älteren Mannes handelt, mit einem Maximum der Inzidenz in der Altersgruppe der 70- bis 74-Jährigen. Außer dem Alter sind nur wenige wesentliche Risikofaktoren bekannt. Männer schwarzafrikanischen Ursprungs sind häufiger betroffen als Europäer oder weiße Nordamerikaner. Am PCa erkrankte nahe Angehörige erhöhen das Erkrankungsrisiko. Eine Keimbahnmutation im BRCA1 oder -2 Gen z. B. erhöht das Erkrankungsrisiko. Umwelt und Lebensstil könnten eine Rolle in der Krankheitsentstehung spielen, jedoch liegt hier nur wenig hochwertige Evidenz vor. Im Vergleich zur Früherkennung kommt der primären Prävention aus diesem Grund aktuell noch eine geringe Bedeutung zu.
