Zusammenfassung

Die Reduktion von Mehrlingen (MFPR) hat sich im Kontext der assistierten Reproduktion entwickelt, um insbesondere das Risiko der Frühgeburt und der damit verbundenen perinatalen Morbidität und Mortalität zu senken. Der Eingriff ist mit medizinischen, psychosozialen und ethischen Problemen behaftet und sollte daher nur nach umfangreicher Aufklärung durch erfahrene Zentren durchgeführt werden. Die Durchführung wird zwischen der 12. und 14. SSW, nach Evaluierung der fetalen Anatomie inklusive der Beurteilung der fetalen Nackentransparenz empfohlen. Eine Reduktion höhergradiger Mehrlinge verringert insbesondere die Frühgeburtenrate <28 SSW und <33 SSW. Bei Drillingen ist dieser Vorteil gegen die höhere Abortrate nach MFPR im Vergleich zum exspektativen Vorgehen abzuwägen. Durch kontrollierte Stimulationsprotokolle im Rahmen der assistierten Reproduktion und den Transfer von nur 1 oder 2 Embryonen kann die Inzidenz höhergradiger Mehrlinge weitestgehend vermieden werden.
