Zusammenfassung
Bei einem akuten Skrotum handelt es sich um einen Symptomenkomplex bestehend aus einer Rötung, Schmerz und Schwellung einer oder beider Skrotalhälften. Das akute Skrotum ist immer als Notfall zu betrachten, da eine Hodentorsion als mögliche Ursache unverzüglich operativer Therapie bedarf. Dennoch kommt diese in lediglich 10–30 % der Fälle vor. Herausforderung bei der Behandlung des akuten Skrotums ist also nach wie vor das Erkennen der richtigen Differenzialdiagnose, ohne zu häufig operativ explorieren zu müssen. Klinisch bewährt hat sich der sog. TWIST-Score (Testicular Workup for Ischemia and Suspected Torsion) mit einem hohen Vorhersagewert für oder gegen das Vorliegen einer Hodentorsion. Dabei werden die klinischen Zeichen bzw. Befunde wie Hodenschwellung, harte Konsistenz, fehlender Kremasterreflex, hohe Skrotallage sowie begleitende Übelkeit oder Erbrechen beurteilt. Die Doppler-Ultraschalluntersuchung der Hoden als zusätzliche apparative Untersuchungstechnik gehört heute zum klinischen Alltag in der Differenzialdiagnostik.
