Zusammenfassung
Bei den Porphyrien handelt es sich um eine klinisch, biochemisch und genetisch heterogene Gruppe metabolischer Erkrankungen. Sie resultieren aus einer vorwiegend hereditären, seltener erworbenen Dysfunktion eines der acht Enzyme der Hämbiosynthese. Während sich die Mehrzahl der verschiedenen Porphyrievarianten mit kutanen Symptomen manifestiert, weisen andere Formen ausschließlich akute, potenziell lebensbedrohliche neuroviszerale Porphyrieattacken auf. Bei allen Porphyrien können Komplikationen an inneren Organen auftreten, hauptsächlich in der Leber und Niere. Daher sollten betroffene Patienten nach Möglichkeit interdisziplinär betreut werden. In diesem Beitrag wird eine aktuelle Übersicht zu Klinik, Diagnostik und Therapie der verschiedenen Porphyrieformen gegeben.
