Akute myeloische Leukämie beim alten und geriatrischen Patienten

Zusammenfassung

Die akute myeloische Leukämie (AML) ist häufigste akute Leukämie im Erwachsenenalter, wobei mehr als die Hälfte der Patienten zum Zeitpunkt der Diagnosestellung älter als 65 Jahre alt sind. Zu Beginn der Behandlung eines älteren AML-Patienten ist die Auswahl einer geeigneten Primärtherapie von entscheidender Bedeutung. Hierbei müssen neben dem Wunsch des Patienten auch patientenbezogene sowie krankheitsspezifische Faktoren berücksichtigt werden. Für die individuelle Prognoseabschätzung steht heute eine Vielzahl von Scores zur Verfügung, die für die Einschätzung des Patienten im Sinne eines geriatrischen Assessments sowie für die Risikobeurteilung der vorliegenden AML-Erkrankung Anwendung finden. Auch wenn die optimale Erstlinientherapie für ältere Patienten mit einer AML weiterhin unklar ist, profitieren prinzipiell auch ältere Patienten von einer intensiven Therapie, sofern sie sich hierfür eignen. Alternativ stehen nicht-intensive und rein supportive Therapieoptionen zur Verfügung.
