Zusammenfassung
Bei Patienten mit systemischem Lupus erythematosus entwickelt sich häufig eine klinisch evidente Lupusnephritis. Dabei finden sich Erythozyturie mit Akanthozyten und Erythrozytenzylindern, Proteinurie bis hin zum nephrotischen Syndrom, eine eingeschränkte glomeruläre Filtrationsrate und eine Hypertonie. Lupusnephritis kann nach den Kriterien der International Society of Nephrology/Renal Pathology Society (ISN/RPS) von 2004 in sechs Klassen unterteilt werden. Die Diagnostik besteht u. a. aus Laboruntersuchungen, bildgebenden Verfahren und der Nierenbiopsie. Die Krankheitsaktivität sollte mit einem Aktivitätserfassungstest dokumentiert werden. Die aktive Lupusnephritis erfordert im Stadium III, IV und V eine immunosuppressive Induktionstherapie und eine deeskalierte Erhaltungstherapie. Patienten- und Nierenüberleben liegen unter immunsuppressiver Therapie bei etwa 90–95 % in 10 Jahren.
