Zusammenfassung

Unter Dyspepsie werden auf den Oberbauch bezogene Beschwerden wie Schmerz, Druck- und Völlegefühl verstanden. Zu den wichtigsten Differenzialdiagnosen der Dyspepsie gehören das peptische Ulkus und die gastroösophageale Refluxerkrankung. Wenn sich keine die Symptome erklärenden Pathologien nachweisen lassen und gleichzeitig die Rom-III-Kriterien in Bezug auf Art, Häufigkeit und Dauer der Beschwerden erfüllt sind, spricht man von einer funktionellen Dyspepsie. Der klinische Befund ist bis auf einen möglicherweise vorhandenen epigastrischen Druckschmerz in der Regel unauffällig. Die Diagnose einer funktionellen Dyspepsie kann nur durch Ausschluss organischer Läsionen gesichert werden. Eine kausale Behandlung existiert nicht und eine dauerhafte Beschwerdefreiheit ist nur bei einer Minderheit der Patienten zu erreichen. Die Therapie zielt auf die Kontrolle der Symptome und die Verbesserung der Lebensqualität. Die Wahl der medikamentösen Therapie richtet sich zunächst nach der dominierenden Symptomatik.
