Zusammenfassung
Der Begriff neuroendokrine Neoplasie (NEN) umfasst eine heterogene Gruppe von Tumoren, die lediglich 0,49 % aller Malignome ausmachen und von äußerst variabler Dignität sind. Bei den rektalen neuroendokrinen Neoplasien handelt es sich meist um hochdifferenzierte neuroendokrine Tumoren (NET). Diese werden häufig inzidentell gefunden, sind asymptomatisch, prognostisch meist günstig und zeigen in der Regel keine funktionelle (endokrinologische) Aktivität. Eine durch den Tumor verursachte Obstruktion des Rektums ist selten. Neben der initialen vollständigen körperlichen Untersuchung sowie der ausführlichen Anamnese wird ein Standardlabor inklusive dem Tumormarker Chromogranin A sowie dem pankreatisches Polypeptid abgenommen. Die Mehrheit der rektalen neuroendokrinen Neoplasien werden mittels endoskopischer Biopsie diagnostiziert. Die einzige kurative Option ist die komplette Resektion im lokalisierten Stadium. Sowohl die Therapie als auch die Nachsorge richten sich nach einer Risikostratifizierung, die dem Metastasenrisiko entspricht.
