Zusammenfassung
Eine Infektion mit dem Hepatitis-Delta-Virus (HDV) tritt ausschließlich gemeinsam mit einer chronischen Hepatitis-B-Virus-Infektion auf (HBV-HDV-Koinfektion). Die Übertragung geschieht entweder gleichzeitig (Simultaninfektion) oder nacheinander (Superinfektion). Weltweit sind 10–15 Millionen Menschen chronisch mit HDV infiziert. Die Simultaninfektion führt meist zu einer akuten Hepatitis B und/oder akuten Hepatitis D mit oft selbstlimitierendem Charakter. Die Superinfektion eines HBV-Trägers mit HDV führt in der Regel nach einer kurzen Inkubationszeit zu einer schweren akuten Hepatitis und wird in bis zu 80 % der Fälle chronisch. Jeder Patient mit einer nachgewiesenen Hepatitis-B-Virusinfektion sollte zumindest einmalig auf das Vorhandensein einer Hepatitis Delta getestet werden. Zusätzlich zur Bestimmung von Anti-HDV und ggf. HDV-RNA sollten die serologischen und virologischen Untersuchungen des Hepatitis-B-Virus erfolgen. Für die Behandlung der chronischen Hepatitis D kommen Interferon alpha infrage.
