Definition

Squamous cell papillomas are the most common type of papilloma occurring in the bronchus, consisting of connective tissue cores covered by regular squamous epithelium.

Squamous cell papillomas can either be single or multiple (papillomatosis) and can display either an inverted or exophytic growth pattern. Solitary papillomas may progress to papillomatosis. Juvenile or adult onset recurrent respiratory papillomatosis (RRP) is defined as numerous squamous papillomas of the respiratory tract caused by human papilloma virus (HPV) infection. This is mostly confined to the larynx, but distal involvement can occur, with lung involvement in less than 1% of cases (Cook et al. 2000).

Clinical Features

One third of these lesions are incidental findings; however, they most commonly present as obstructing lesions. Presenting symptoms include cough, wheeze, recurrent pneumonia, lobar collapse, and hemoptysis. They may be found as an incidental radiological finding.
