Definition

Chondrodermatitis nodularis chronica helicis (CNCH) is an idiopathic non-neoplastic inflammatory and degenerative process of the external ear characterized by necrobiotic changes in the dermis which extend down to the perichondrium, with associated alterations of the cartilaginous plate.

Etiology is not well established. According to original description by Winkler from 1915, there was an association to frostbite. This opinion, still held by many, is not supported by recent publications. It seems most likely that CNCH is related to diminished/poor blood supply at the periphery of the auricle. The lack of a thick, cushioning, subcutaneous layer in the auricle may predispose the underlying cartilage to pressure-induced ischemia and damage.
