Definition
Concrescence describes fusion of two or more teeth by deposition of cementum following completion of root formation. It tends to occur most frequently in the maxilla with fusion of the roots of the second and third molars (Levitas 1965). Anatomically the root apices are immediately adjacent; therefore, any degree of hypercementosis may bridge the gap between the two roots and fusion of the cementum can occur. This may not come to light until extraction of one of these teeth is attempted. Pressure exerted on the tooth to be extracted is transmitted to the adjacent tooth which may make the extraction difficult. Alternatively the force which is applied may cause both teeth to become mobile, sometimes together with the surrounding bone. In this way, the tuberosity of the maxilla can become fractured, and this may in turn expose the maxillary antrum and lead to the creation of an oroantral fistula.
