The hand and wrist are a complex anatomic construction and heavily used part of the body. Consequently, disorders caused by overuse, degeneration, infection, inflammation, and trauma are frequent. In the following chapters, we focus on the conditions, where bone SPECT/CT may play a role in the diagnosis and therapy and will present illustrative case examples. In general, SPECT/CT of the wrist and hand can serve as an advanced imaging technique and problem-solving tool, if conventional x-rays and CT are insufficient or inconclusive, for example, in occult wrist fracture (Huellner et al. 2012). SPECT/CT has shown significant impact on therapy in more than one-third of patients with unspecific hand/wrist pain (Schleich et al. 2012). SPECT/CT can serve as a valuable alternative in patients with contraindications for MR imaging. In patients with implanted metal hardware, SPECT/CT might be superior to MR because of lesser vulnerability to metal-induced artifacts.
