Abstract

Conventional radiography is the standard imaging modality for routine follow-up after hip arthroplasty. Other imaging methods should be used only in case of prolonged or progressive symptoms. Bone SPECT/CT can accurately detect or rule out loosening of hip arthroplasty, heterotopic ossifications, and recurring pain following hip arthroplasty, but knowledge of the expected changes in bone turnover after hip arthroplasty is required. Due to inflammation, vascular and bloodpool uptake remains increased in the first 3 months following surgery, and the delayed-phase images take even longer to normalize. Typically, osteointegration takes longer in uncemented stems, and residual uptake can last for years in the greater trochanter and at the stem tip. This chapter describes the postoperative physiological evolution of hip arthroplasties on bone SPECT/CT.
