Greater Trochanteric Pain Syndrome (GTPS) and Iliopsoas Bursitis

Abstract

The greater trochanteric pain syndrome (GTPS) (previously termed trochanteric bursitis) is a clinical syndrome defined by chronic lateral hip pain. In contrast, iliopsoas bursitis causes anterior hip and groin pain. Both entities are caused by repetitive trauma or overuse and involve inflammation of the hip’s bursae. While ultrasound is the preferred first-line imaging test to diagnose bursitis, many other conditions can produce similar symptoms, and bone SPECT/CT can rule out other pain generators and assess the involvement of entheses in the inflammatory process. In addition, careful assessment of the CT part of the study can provide clues to the coexistence of bursitis and impact subsequent workup and management. This chapter discusses the role of different imaging modalities in diagnosing GTPS and iliopsoas bursitis.
