Postoperative Spine: Epidural Fibrosis

Abstract

Epidural fibrosis can cause compression or tethering of the nerve root and has been associated with failed back surgery syndrome (FBSS). However, the clinical relevance of imaging findings of epidural fibrosis is controversial. While bone SPECT/CT is not indicated to diagnose epidural fibrosis, the CT part of the study often allows an initial assessment of the presence of this postoperative complication. This chapter describes a case of FBSS where bone SPECT/CT identifies multiple possible pain generators and illustrates the relevance of inspecting the CT part of the study in the soft-tissue window to screen for epidural fibrosis.
