Abstract

Metastatic disease, myeloma, and lymphoma often present as multiple lesions in the spine, making the diagnosis of malignancy straightforward. In contrast, unifocal spine lesions may cause diagnostic uncertainty, and the differentiation between benign and malignant pathology can be challenging. This chapter discusses the imaging characteristics of common spine lesions and presents a classification system that may assist in differentiating aggressive from nonaggressive lesions. In addition, the Bone-RADS framework is introduced to help guide the management of patients with a solitary bone lesion based on its imaging features.
