Population Health for Older Adults

Abstract

To describe population health, one needs to start with understanding “why” population health exists. What are the elements that have come together to create population health today? The answer to “why” population health exists and is relevant today is that today’s health care providers are faced with two significant challenges and market shifts that have resulted in the need for population health. Basically, a new health care supply and demand dynamic have produced population health.
