Metastatic Orbital Lesions: Melanoma

1 Signs and Symptoms

Signs and symptoms are similar to that elaborated in “Metastatic Orbital Lesions: Breast Cancer”. Orbital metastases from cutaneous melanoma have particular affinity to the extraocular muscles, and diplopia is a common presenting symptom. Proptosis and blepharoptosis are equally common presenting signs. Cutaneous melanoma can also send choroidal metastasis that may mimic uveal melanoma. Bilateral orbital involvement and multiple orbit and eyelid metastasis can occur in relatively low percent.

2 Differential Diagnosis

Similar to the differential in “Metastatic Orbital Lesions: Breast Cancer”, these metastases are commonly seen as smooth enlargement of an extraocular muscle rather than as general infiltration to the orbit.

3 Diagnosis

Diagnosis is made via an orbital biopsy of the suspected lesion. A known primary cutaneous melanoma/metastatic disease may raise the suspicion of orbital involvement. The trunk is the most common primary site to send orbital metastasis...
