Abstract

Obesity is an incompletely understood disease as are the mechanisms of action of bariatric operations on both weight loss and the treatment of associated comorbidities. There are multiple different procedures available, each with their own merits and complications profile. When combined with highly variable patient factors (not just physical constraints), the choice of procedure for the individual patient can be complex. This chapter discusses the decision-making involved in performing a bariatric procedure for the individual patient. It provides an overview of the benefits and pitfalls of the most commonly performed bariatric procedures (including endoscopic techniques) and examines how individual, surgical, and regulatory factors affect the type of surgery selected. This will help the surgeon discuss surgical options with each patient, outlining the potential positive and negative aspects of each procedure for that individual.
