Gastrointestinal Endoscopy: Considerations

Abstract

Despite the increasing percentage of the older population, there is still little literature on gastrointestinal endoscopy in this patient group. Importantly, because of population demographics, the use of endoscopy is expected to rise in this section of the population worldwide. Older adults represent a special group, as aging results in changes in pathophysiology of the gastrointestinal tract. A substantial significant percentage of geriatric patients suffer from comorbid diseases. The spectrum of gastrointestinal disorders differs from that in younger patients and the elderly are, in general, more susceptible to complications from therapeutic endoscopic interventions. Indeed, due to the decreased physiologic reserve and associated diseases, complications in this age group tend to be more severe than in subjects younger than 65 years. Moreover, ethical considerations play a special role in frail patients with a potential poor prognosis, where the indication for endoscopy may be correct, but conducting endoscopy may not lead to a better prognosis. Thus, the endoscopist needs to pay special attention to the geriatric patient, indication and potential benefit when considering or performing endoscopy.

In this chapter, we review the role of endoscopy in the geriatric patient, with emphasis on the perioperative period, including pre-endoscopic assessment, management of anticoagulants, sedation, endoscopic procedures, and post-endoscopic care.
