Transfusion-Related Acute Lung Injury (TRALI)

Abstract

TRALI is an adult respiratory distress syndrome occurring within 6 h of transfusion that presents with fever, cyanosis, dyspnea, and opacity of the pulmonary fields on frontal chest radiograph. TRALI has been described with all blood components, and its mechanism has been associated with donor HLA antibodies and some biologic response modifiers that trigger a leukocyte damage of the lung capillaries leading to leakage of fluid in the alveolar spaces. There is a 20% mortality as consequence of TRALI, and many patients require oxygen support and mechanical ventilation.
