Zusammenfassung
Die operative Korrektur von Deformitäten der Beinachse mit externen Fixateuren ist kein neues Verfahren. Auch wenn moderne Hexapoden-Fixateursysteme vermuten lassen, die Entwicklung sei jung und das Verfahren hinreichend komplex in der Anwendung. Fixateure sind in der Nutzung bei traumatologischen Eingriffen weit verbreitet; kaum ein Anwender denkt darüber nach, dass sie vor 40–50 Jahren eher das Werkzeug für die Deformitätenkorrekturen waren. Dieser Beitrag will grundsätzliche Ideen der äußeren Knochenspanner verdeutlichen und Verständnis bei Anwendern von Platten für die hohe tibiale Osteotomie (HTO) und die distale Femurosteotomie (DFO) wecken. Anhand von Fallbeispielen kindlicher Deformitäten soll der heutige elektive Anwendungsnutzen in der operativen Deformitätenkorrektur verdeutlicht werden.
Abstract
Surgical axis correction of deformities of the leg with external fixators is not new, even if modern hexapod fixator systems suggest that the development is recent and the procedure is sufficiently complex to use. Fixators are widespread in the use for traumatological interventions. Hardly any user thinks about the fact that 40–50 years ago they were more of a tool for deformity corrections. This article aims to highlight the basic ideas of external bone tensioner and to arouse understanding among users of plates for high tibial osteotomy (HTO) and distal femoral osteotomy (DFO). Based on case examples of deformities in children, the current benefits of elective application of fixators in surgical correction of deformities are elucidated.
