Zusammenfassung
Einleitung
Seit 2013 ist im Bachelor-Studiengang Gesundheitsökonomie der Hochschule RheinMain ein verpflichtendes Krankenpflegepraktikum von 2 Monaten Dauer vorgesehen. Dies stellt eine gefahrengeneigte Tätigkeit mit Risiken für Patienten und Studierende dar. Daher wurde eine Risikobeurteilung durchgeführt, um durch geeignete Maßnahmen diese Risiken zu minimieren und andernfalls entstehende Haftungsrisiken für die Hochschule zu berücksichtigen.
Methoden
Mittels Szenario-Analyse nach ONR 49002-2 wurden die wesentlichen Risiken für die Patienten- und Mitarbeitersicherheit unter Annahme des „credible worst case“ beurteilt. Auf Basis dieser Analyse wurden Präventionsmaßnahmen vor Beginn der Entsendung der ersten Kohorte in ein Pflegepraktikum abgeleitet, umgesetzt und Maßnahmen zur Überprüfung der Wirksamkeit festgelegt.
Ergebnisse
Fünf Risiken wurden als relevant und als in dieser bisherigen Ausprägung ohne weitere Intervention nicht tragbar angesehen: nosokomiale Infektionen, Verzögerung einer Notfallbehandlung, Nadelstichverletzungen (NSV), mangelhafte Beachtung bzw. Umsetzung von Brandschutzmaßnahmen und fehlerhafte Handlungen im Brandfall sowie Übernahme überfordernder, unbeaufsichtigt durchgeführter Tätigkeiten. Basierend auf der Risikoanalyse wurden eine arbeitsmedizinische Vorsorge, eine Kurzzeitlehrintervention vor Aufnahme der praktischen Tätigkeit und eine schriftliche Belehrung etabliert.
Diskussion
Durch die präventive, systematische Risikobeurteilung konnten die wesentlichen Risiken durch anerkannte Präventionsmaßnahmen für alle Betroffenen minimiert werden. Die Anwendung der Szenario-Technik erwies sich hierbei als hilfreich und sollte bei der Beurteilung komplexer Risiken auch abseits von Gefährdungsbeurteilungen in Betracht gezogen werden.
Abstract
Introduction
In 2013 the RheinMain University of Applied Sciences launched its bachelor program in health care economics, which included a mandatory 2‑month nursing internship; however, this internship may be associated with several relevant risks for the safety of patients and students. Prior to the first nursing internships a systematic risk assessment was performed to minimize these risks and to address the liability risks for the University.
Methods
Based on a scenario analysis according to Austrian Standards Institute Rule (ONR) 49002-2, risks caused by this nursing internship were identified and assessed assuming credible worst-case scenarios. Preventive measures were derived, implemented and evaluated as far as possible prior to sending out the first cohort of students to nursing internships.
Results
A total of five relevant risks were identified and addressed by preventive measures: nosocomial infections, delays in treatment of medical emergencies, needle stick injuries, deficient attention to or implementation of fire protection measures and inappropriate behavior in case of fire as well as taking on too demanding unsupervised tasks. Based on this risk assessment preventive measures were implemented consisting of occupational medical health precautions, a short-term educational intervention and written instructions about boundaries of competencies during nursing internships.
Discussion
The preventive systematic risk assessment enabled identification and minimization of relevant risks related to nursing internships of the students. The scenario technique proved to be very helpful and should be taken into consideration for assessment of complex risks in general.
