Samenvatting
Introductie:
Om de kwaliteit van hypospadiechirurgie te meten en zo mogelijk te verbeteren, is in 2009 gestart met de Nederlandse Hypospadie Studie. Dit artikel beschrijft de opzet van de studie en de eerste kortetermijnuitkomsten (= 6 maanden postoperatief) ten aanzien van opgetreden complicaties en ten aanzien van het cosmetisch resultaat.
Patiënten en methoden:
In een onlinedatabase worden gegevens vastgelegd van de patiënt, de aandoening, de operatietechnische details, van complicaties die zijn opgetreden en foto’s van het cosmetisch uiterlijk. Ouders geven op een tienpuntsschaal een cijfer voor het algeheel cosmetisch uiterlijk preen (6 maanden) postoperatief. Deelnemende operateurs krijgen jaarlijks een terugkoppeling van hun eigen resultaten ten opzichte van die uit zijn/haar centrum en het landelijk gemiddelde. Op termijn zullen prognostische factoren en operatietechnische details van best practices worden gerapporteerd, op grond waarvan de deelnemers desgewenst hun operatietechniek kunnen aanpassen.
Resultaten:
Per medio oktober 2012 zijn 909 patiënten geïncludeerd; 797 patiënten zijn geopereerd van wie er 654 zes maanden postoperatief evalueerbaar waren. De patiënten werden ingedeeld in 3 groepen: groep 1 had glandulaire hypospadie (25%), groep 2 (sub)coronaire, distale en midschacht hypospadie (67%) en groep 3 proximale schacht en penoscrotale hypospadie (8%). Het gemiddelde complicatiepercentage in groep 1, 2 en 3 was respectievelijk 6%, 20% en 30%. Het gemiddelde cijfer voor cosmetisch resultaat nam in groep 1, 2 en 3 respectievelijk toe met 2,5, 2,5 en 2,4 punten. De spreiding in complicaties en cosmetisch resultaat tussen de operateurs geeft aan dat het kenbaar maken van operatietechnische details van de best practice(s) kan leiden tot kwaliteitsverbetering.
Conclusie:
De Nederlandse Hypospadie Studie registreert de uitkomsten van hypospadiechirurgie; daarnaast is de verwachting dat de kwaliteit van de hypospadiebehandeling in Nederland verder kan verbeteren door rapportage van prognostische factoren en de best practices.
Summary
Dutch Hypospadias Study: design and preliminary results
Introduction:
In 2009 the Dutch Hypospadias Study Group initiated a multicenter study to evaluate and possibly improve the outcomes of hypospadias surgery. This article describes the study design and the first short term (six months postoperatively) outcomes of complications and cosmetic result.
Patients and methods:
Data of the patient, disorder, surgical technique, complications and photographs of cosmetic appearance are documented in a web based database. Parents determined the general cosmetic appearance pre- and (six months) postoperatively on a tenpoint scale. Annual reports inform the participating hypospadias surgeons about their own results, mean outcomes of their department and the total multicenter study population. Prognostic factors and surgical details of best practices will be reported in the future enabling the participants to adapt their operative technique.
Results:
In October 2012, 909 patients are enrolled in this study and 797 patients are surgically corrected of whom 654 are evaluated six months postoperatively. These hypospadias patients are classified in three groups: group 1 glanulair hypospadias (25%), group 2 (sub)coronal, distal and midshaft hypospadias (67%) en group 3 proximal shaft and penoscrotal hypospadias (8%). Mean complication rate in group 1, 2 and 3 is 6%, 20% and 30% respectively.
Postoperative improvement of the general cosmetic appearance in group 1, 2 and 3 is 2.5, 2.5 and 2.4 points respectively. Distribution of complication rate and cosmetic result between hypospadias surgeons indicates outcomes may improve by reporting surgical details of best practices.
Conclusion:
The Dutch Hypospadias Study registers surgical results and is expected to improve the quality of outcomes by reporting prognostic factors and best practices.
