Résumé
La prise en charge du paludisme grave est un défi majeur dans les services de soins en Afrique sub-saharienne. Cette étude porte sur l’efficacité et la tolérance de l’artémether et la quinine sur le paludisme grave au Complexe pédiatrique de Bangui en République centrafricaine. Un total de 212 enfants atteints de paludisme grave et âgés de 6 à 59 mois a été inclus dans cette étude (108 traités par de l’artémether et 104 enfants traités par de la quinine) sur 1 125 hospitalisations, soit une proportion de 18,8 %. L’anémie (58,5 %) et les convulsions (33,5 %) étaient les principaux syndromes observés. Au troisième jour de prise en charge, une régression des signes clinique et une clairance parasitaire ont été observées dans 98,1 % des enfants traités par de l’artémether et dans la proportion de 97,1 % des enfants traités par de la quinine. La létalité a été de 2,3 % due aux formes anémiques et neurologiques. Ces résultats montrent que l’artémether et la quinine ont une efficacité similaire. Associée aux soins adéquats, l’utilisation de ces médicaments permet de réduire significativement la mortalité du paludisme grave.
Abstract
The management of severe malaria is a major challenge in the health care services in sub-Saharan Africa. This study aimed to assess the efficacy and safety of artemether and quinine in severe malaria at Complexe pédiatrique of Bangui, Central African Republic. A total of 212 children among 1125 hospital admissions (18.8%), and aged 6 to 59 months were randomly treated with artemether and quinine. Anemia (58.5%) and seizures (33.5%) were the major syndromes observed. On the third day of follow up, a regression of clinical signs and parasite clearance were observed in 98.1% of children treated with artemether and 97.1% of children treated with quinine. The death rate was 2.3% due to anemic and neurological forms. These findings show that the artemether and quinine have similar efficacy. Hence, associated with adequate intensive health care, the use of these antimalarial drugs can significantly reduce mortality from severe malaria in the Central African Republic.
