In de Standaardvraagstelling Angst & Depressie van de Lokale en Nationale Monitor Volksgezondheid is door epidemiologen van GGD’en afgesproken om de Kessler Psychological Distress Scale (K10) te gebruiken voor het schatten van de prevalentie van depressie en angststoornissen in hun werkgebied. Dit artikel laat zien dat toepassing van deze screener resultaten oplevert die aanzienlijk afwijken van de prevalentieschattingen op basis van het Composite International Diagnostic Interview (CIDI) dat als gouden standaard geldt. Aan de hand van data uit een studie waarin zowel de K10 als de CIDI bij een groep respondenten is afgenomen, wordt gëıllustreerd dat de K10 niet bruikbaar is voor het schatten van de prevalentie. Omdat het schatten van de prevalentie met de gouden standaard (CIDI) voor GGD’en vanwege de hoge kosten niet altijd haalbaar is, wordt afgesloten met het beschrijven van twee alternatieve methoden om de prevalentie te schatten.
Abstract
The usefulness of the Kessler Psychological Distress Scale (K10) for estimating the prevalence of depression and anxiety disorders
Dutch regional public health epidemiologists have agreed to use the Kessler Psychological Distress Scale (K10) to estimate the local prevalence of depression and anxiety disorders. This article demonstrates that using the K10, yields outcomes which deviate substantially from results generated by the CIDI (golden standard). Using a sample in which both the K10 and CIDI were administered, shows that the K10 is not appropriate for estimating the prevalence. Because using the CIDI for estimating the prevalence is not always feasible for regional public health institutions on account of the high costs, we conclude with a description of two alternative methods for prevalence estimation.
