ACHTERGROND Een van de zorgvuldigheidseisen bij euthanasie is toetsing door een onafhankelijke arts. In 1999 is daarom onder verantwoordelijkheid van de KNMG het netwerk Steun en Consultatie bij Euthanasie in Nederland (SCEN) gevormd. Wij onderzochten waarin goede SCEN-consultaties zich onderscheiden van minder goede SCEN-consultaties.

METHODE Wij interviewden 14 SCEN-artsen, 12 consultvragende artsen en 10 nabestaanden van patiënten bij wie een SCEN-consultatie had plaatsgevonden. Daarnaast ontvingen wij ingevulde vragenlijsten van 508 SCEN-artsen (respons 86%) en 13 leden van Regionale Toetsingscommissies euthanasie (respons 87%). Dit vulden wij aan met informatie uit 80 consultatieverslagen.

RESULTATEN De respondenten zijn het eens over wat een goede SCEN-consultatie kenmerkt, zowel procedureel (de consultvragende arts spreken vóór het bezoek aan de patiënt) en inhoudelijk (onafhankelijk beoordelen van de zorgvuldigheidseisen) als communicatief (zich empathisch opstellen naar de patiënt). Als een consultatie minder goed verloopt, komt dat vaak door praktische problemen bij de uitvoering. Belangrijk daarbij is het tijdstip: bij een ‘vroege’ consultatie is de patiënt wel aanspreekbaar, maar kan de objectiviteit in het geding komen.

CONCLUSIE Er zijn nog verbeteringen mogelijk binnen SCEN. Duidelijke richtlijnen over het tijdstip van consultatie en nascholing op het gebied van wetgeving en palliatieve zorg zijn in het oog springende punten. De begeleidingsbijeenkomsten kunnen een belangrijk instrument zijn voor de implementatie van dit kwaliteitsbeleid.

BACKGROUND One of the preconditions for euthanasia is evaluation by or consultation with an independent doctor. To this end, the Royal Dutch Medical Association set up a specialized service, SCEN (Support and Consultation for Euthanasia in the Netherlands), in 1999. We investigated how good SCEN consultations differ from less satisfactory SCEN consultations.

METHODS We interviewed 14 SCEN doctors, 12 doctors who had requested a euthanasia consultation, and 10 relatives of patients who had had a SCEN consultation. In addition, a questionnaire was sent to all SCEN doctors in the Netherlands (n = 594, response 86%) and to 13 members of regional Euthanasia Review Committees. Additional information was retrieved from 80 consultation reports of SCEN doctors.

RESULTS The respondents agreed about what characterized a good SCEN consultation, for example procedural aspects (such as talking to the doctor who had requested a consultation before visiting the patient), consultation content (such as independently evaluating whether requirements of prudence are fulfilled), and communication skills (such as adopting an empathic attitude towards the patient). Less satisfactory consultations are often due to practical problems, such as the timing of the consultation: patients may be able to communicate when consultations are held in an early stage, but objectivity may be compromised.

CONCLUSION The SCEN procedure could be improved. For example, the timing of the consultation and refresher courses on legal aspects and palliative care are important points to consider. Compulsory ‘intervision’ meetings may be an important instrument for quality control of this policy.
