Pathology of odontogenic lesions in the maxillary sinus

Zusammenfassung

Odontogene Kieferhöhlenerkrankungen können in der differenzialdiagnostischen Abgrenzung signifikante Schwierigkeiten bereiten. Die korrekte histopathologische Einordnung ist aber unerlässliche Voraussetzung für eine adäquate Therapie, die zwar i.d.R. in einem chirurgischen Eingriff besteht, aber von einer minimal-invasiven Maßnahme bis zu einer kontinuitätsunterbrechenden En-bloc–Resektion mit notwendigen Sicherheitsabständen und ggf. Lymphknotendissektion reichen kann. Nur durch die Integration klinischer, radiologischer und histopathologischer Befunde kann eine ausreichende Entscheidungsbasis für eine adäquate individuelle Therapie sichergestellt werden. In diesem Beitrag werden wichtige Entitäten, die es in histopathologischer Hinsicht voneinander abzugrenzen gilt, in ihrer klinischen Bedeutung dargestellt.

Abstract

The differential diagnosis of odontogenic lesions of the maxillary sinus may cause significant problems; however an adequate therapy is based on the correct histopathological diagnosis determining whether the surgical treatment consists of a simple minimally invasive procedure or a complex resection with safety margins and perhaps also lymph node dissection. Only an integrated approach considering all clinical, radiological and histopathological information can allow for an adequate treatment of an individual case. This overview discusses the most relevant entities which can be differentiated by histopathological criteria with respect to their clinical impact.
