Zusammenfassung

Der mikrovaskuläre Gewebetransfer hat sich seit seiner Einführung vor etwa 30 Jahren schnell zu einer Routinemethode entwickelt, die heute im Zentrum der rekonstruktiven Verfahren auch in der MKG-Chirurgie steht. Der Beschaffenheit der Defektregion entsprechend können Transplantate mit verschiedenen Gewebekomponenten an zahlreichen Entnahmestellen gehoben werden, die eine konstante Anatomie aufweisen und somit standardisierbare Präparationstechniken erlauben. Als eine Erweiterung dieser gut etablierten Techniken werden in neuerer Zeit auch sog. Perforanslappen diskutiert. Diese zeichnen sich durch eine minimale Entnahmemorbidität aus, machen allerdings aufgrund der variantenreichen Anatomie oftmals individuelle Vorgehensweisen bei der Lappenhebung erforderlich. Der mikrovaskuläre Gewebetransfer ist unverzichtbarer Bestandteil speziell der Tumorchirurgie geworden. Seine Erfolgsraten werden von größeren Zentren mit 95% angegeben. Er kann somit als ein sehr sicheres Rekonstruktionsverfahren in der MKG-Chirurgie gewertet werden.

Abstract

Microvascular tissue transfer has rapidly developed to a routine method since is introduction approximately 30 years ago and is nowadays also the focal point of reconstructive procedures in oral and craniomaxillofacial surgery. Depending on the condition of the defect area, transplants with various tissue components can be acquired from many donor sites, which are distinguished by a constant anatomy and therefore allow standardized preparation techniques. Recently, the so-called perforator flaps have been discussed as an extension of this well-established technique and are characterized by minimal donor morbidity but often make an individual approach for flap raising necessary due to the wide variation in anatomy. Microvascular tissue transfer has become an indispensible component particularly in tumor surgery and success rates of 95% have been cited by large centers. It can therefore be considered an extremely safe reconstruction procedure in oral and craniomaxillofacial surgery.
