Déterminer la prévalence des différents cancers et lésions précancéreuses gastriques.
Méthode
Etude rétrospective sur 5 ans (2003–2008) à partir des biopsies gastriques prélevées par voie endoscopique dans tous les établissements pratiquant l’endoscopie digestive haute à Bujumbura et analysées au laboratoire de référence du CHU de Kamenge.
Résultats
Durant cette période, nous avons colligé 710 dossiers de malades qui avaient des comptes-rendus complets, contenant toutes les données requises pour ce travail. 415 étaient des hommes (58,4%) et 295 des femmes (41,6%). L’âge moyen était de 49,1 ± 10 ans avec les extrêmes de 12 et 88 ans. Les indications de l’endoscopie étaient dominées par les douleurs épigastriques (92,6%). Les résultats anatomo-pathologiques étaient: la gastrite chronique chez 435 patients (61,3%). Elle était atrophique chez 203 malades (28,6%) et lymphocytaire chez 4 patients (0,6%). Les autres lésions étaient l’ulcère gastrique chronique chez 115 malades (16,2%), les polypes gastriques adénomateux chez 3 patients (0,4%), la maladie de Ménétrier chez 3 patients (0,4%), la métaplasie intestinale chez 21 patients (3%), les dysplasies sévères chez 59 malades (8,3%), l’adénocarcinome chez 51 patients (7,2%), un lymphome malin non hodgkinien chez 5 patients (0,7%) et un sarcome de Kaposi chez 1 patient (0,1%). L’Helicobacter pylori a été recherché chez 619 malades et était positif chez 524 (84,7%).
Conclusion
Les cancers gastriques et les lésions précancéreuses sont fréquents au Burundi. Ces lésions commencent à se manifester vers la 4e décennie. Une attention particulière devrait être portée sur des patients ayant l’âge de plus 40 ans pour faire plus de biopsies à la recherche des lésions précancéreuses et prévenir le cancer.

Aim
To estimate the prevalence of gastric cancers and precancerous lesions
Method
This is a retrospective study on 5 years (2003–2008) about gastric biopsies done by upper endoscopy in all Bujumbura Centers that practiced upper digestive endoscopy, and examined in the pathology laboratory of Kamenge university hospital.
Results
During that period, we selected 710 patients’ records that contained complete and required data for this study. 415 were males (58,4%) and 295 females (41,6%). The middle age was of 49,1 ±10years with the extremes of 12 to 88 years. The main indication of the upper endoscopy was epigastric pain (92,6% of patients). As pathology results, chronic gastritis was found in 61,3% (435 patients). It was atrophic in 28,6% (203patients) and lymphocytic in 0,6% (4 patients). Other lesions were chronic gastric ulcer: 16,2% (115 patients), adenomatous gastric polyp: 0,4% (3 patients), Fiddler illness 0,4% (3 patients), intestinal metaplasia: 3% (21 patients), severe dysplasia: 8,3% (59 patients), adenocarcinoma: 7,2% (51patients), primary gastric non hodgkin’s lymphoma: 0,7% (5 patients) and Kaposi sarcoma 0,1% (1 patient). The Helicobacter pylori was investigated for 619 patients and was positive for 524 (84,7%).
Conclusion
The gastric cancers and precancerous lesions are frequent in Burundi. Those lesions appeared in the 4th decade of life. More attention should be done on patients aged over 40 years by doing more biopsies to detect precancerous lesions and prevent the degeneration into a cancer.
