Zusammenfassung
Wir verglichen die Kurven und Meßergebnisse eines neuen ambulant einzusetzenden Screening-Gerätes (SOMNOcheck®, Weinmann, Hamburg; Thermistorflow oder Beatmungsdruck, Schnarchgeräusche, Pulsoxymetrie, Körperlage) mit den Ergebnissen zeitgleicher stationärer Polysomnographien bei 59 Patienten, die mit der Verdachtsdiagnose einer schlafbezogenen obstruktiven Atmungsstörung untersucht wurden. 39 Messungen wurden als Leermessung, 20 Messungen unter nCPAP/BiPAP-Therapie durchgeführt. 4 Messungen waren aufgrund patientenbedingter Sensorartefakte nicht verwertbar. Die Differenzen des polysomnographischen Apnoe-Hypopnoe-Index und des Wertes des Screening-Gerätes betrugen (Mittelwert, Konfidenzintervall) 1 (−1bis+3)/h für Leermessungen bei handausgewertetem Screening und 0 (0bis +1)/h für Messungen unter Beatmung. Das Screening-Gerät ist gut geeignet zur ambulanten Diagnostik schlafbezogener Atmungsstörungen.
Summary
We compared results of polysomnographic measurements with that of an ambulatory monitoring system (SOMNOcheck®, Weinmann, Hamburg; thermistorflow or respiration pressure, snoring, pulse-oximetry, body position) in 59 patients, who were evaluated in a sleep lab for suspected sleep disordered breathing. 39 measurements were performed without and 20 measurements with nCPAP/BiPAP-therapy. 4 measurements were rejected due to patient induced motion artifacts. The differences of apnea-hypopnea-index, deriving during polysomnography and during the screening device respectively, for measurements with thermistor (mean, confidence interval) were 1(−1 to +3)/h, for measurements with nCPAP/BiPAP 0 (0 to 1)/h. We conclude that the ambulatory system produces valid data for out-patient diagnosis of sleep disordered breathing (ASDA-level: less-than-full respiratory recording).
