Abstract
ICD-10 and DSM-IV differ in their definitions of delirium. The DSM-IV definition centers around a disorder of attention and cognitive functions, whereas ICD-10 describes delirium as a broader neuropsychological and psychopathological syndrome, e.g. with hallucinations, emotional and psychomotor disorder.
When neuropsychological and psychopathological symptoms of delirium are assessed simultaneously, our question was, whether there are core symptoms of delirium, i.e. neuropsychological and psychopathological symptoms. Our approach is to identify core symptoms of delirium 1) by a high interrelation, and 2) furthermore core symptoms of delirium should show this interrelation consistently across different etiological groups of delirium.
There are consistent correlations between psychopathological symptoms of delirium and neuropsychological findings of cognitive deficits. These consistent correlations seem to be largely restricted to the domain of the disorder of awareness of the surroundings and are not found in a systematic manner for symptoms in the fields of associated features of delirium like emotional, vegetative or circadian disorders.

Die Diagnostik eines Delirs ist uneinheitlich. Während die ICD-10-Kriterien das Delir als ein komplexes neuropsychiatrisches Syndrom beschreiben, fokussiert das DSMIV-Manual die Diagnostik auf ein Kernsyndrom aus einer Störung der Bewusstheit der Umgebung und einer kognitiven Störung.
Methodik
Es wurden 94 delirante Patienten neuropsychologisch und psychopathologisch untersucht. Unabhängig davon wurden sie drei ätiologischen Gruppen zugeordnet (Alkoholentzugsyndrom, Delir bei Demenz und Delir anderer Ursachen). Die psychopathologischen Symptome und die kognitiven Störungen wurden miteinander korreliert und bezüglich der Ätiologien miteinander verglichen.
Ergebnis
Die Merkmale einer Störung der Bewusstheit der Umgebung korrelieren deutlich höher als die übrigen psychopathologischen Symptome mit den neuropsychologischen Störungen.
Konklusion
Es wird die Bedeutung einer gestörten Kernsymptomatik im Delir bestätigt und die Einordnung der übrigen Psychopathologie als assoziierte Symptome eines Delirs diskutiert.
