Key points

• Despite the excellent surgical outcomes of laparoscopic Roux-en-Y gastric bypass (RYGB), the prevention of complications remains a challenge. The gastrojejunostomy can be a technically challenging step.

• Dissecting a false plane into the gastric submucosa can occur while performing the gastrojejunal anastomosis with linear endostapler.

• While performing a gastrojejunostomy, it is important to properly expose the gastric mucosa in order to include the full thickness of the stomach wall before firing the stapler.
