Zusammenfassung
Standardisierte Qualitätsinformationen ermöglichen es, die Transparenz auf Märkten von Leistungen mit Informationsunsicherheiten zu erhöhen. Die Auswirkungen der Veröffentlichung von standardisierten Qualitätsinformationen für komplexe Dienstleistungen auf die Wahlentscheidungen von Nachfragern wurden bisher in Deutschland nicht untersucht. Die vorliegende Arbeit untersucht den Einfluss von standardisierten Qualitätsinformationen auf die Krankenhauswahl. Dazu werden in zwei Experimenten anhand hypothetischer Entscheidungsszenarios zwei Krankenhäuser verglichen. Die Qualitätsinformationen von einem der Krankenhäuser werden systematisch variiert und die resultierenden Wahlentscheidungen werden analysiert. Die Ergebnisse zeigen die Abhängigkeit der Wahlentscheidungen von den Qualitätsinformationen, deren Präsentation und von zusätzlichen Informationsbestandteilen. Die Bedeutung dieser Ergebnisse für den Qualitätswettbewerb im Gesundheitswesen und für die Gestaltung der Kommunikation von Qualitätsinformationen wird diskutiert.
Abstract
This study analyses the impact of standardized quality information of complex services on decisions, exemplified for Germany’s hospital quality reports. The presumed impact of quality information on decisions is based on theoretical considerations and hypotheses are developed accordingly. Though results of two experiments show that simplified quality indicators used in the quality reports for hospitals basically lead to consistent decisions, results also show that the impact of standardized quality information on decisions is not stable. It is disturbed and superimposed by the effects of additionally presented quality information. Implications for competition on hospital markets and for the effectiveness of standardized quality information as they are used in the hospitals’ quality reports are discussed as well as general conclusions about standardized information of complex services are drawn.

Qualitätsindikatoren zur Ergebnisqualität wurden erst im Jahr 2006 von der BQS verbindlich festgelegt und sind nicht Gegenstand der vorliegenden Studie.
Über Möglichkeiten zur Verbesserung der Veröffentlichung von Qualitätsinformationen im Rahmen der strukturierten Qualitätsberichte wird diskutiert (vgl. Lütticke/Schellschmidt 2005). In der aktuellen Form wurde die Qualitätsberichterstattung für das Berichtsjahr 2006 modifiziert, indem zusätzliche Ergebnisindikatoren, bundesweite Vergleichswerte und die Möglichkeit zur Angabe spezifischer Kompetenzen für die Krankenhäuser ermöglicht wurden (vgl. Gemeinsamer Bundesausschuss 2007; BQS 2007).
Die Szenario-Technik in der experimentellen Forschung wurde z.B. von Smith et al. 1999 im Rahmen der Zufriedenheitsforschung verwendet.
Es wäre auch denkbar, dass die Verschiebung des Referenzpunktes die Funktionsform bzw. die Funktionsparameter der unterstellten Wertefunktion verändern könnte. Demnach wären die Funktionsform bzw. die Funktionsparameter selbst eine Funktion der Höhe des Referenzpunkts. Hier wird hingegen davon ausgegangen, dass die Verschiebung des Referenzpunktes keine solchen Effekte auf die Funktionsform bzw. auf Funktionsparameter hat und sich die zu Grunde gelegte Wertefunktion mit dem Referenzpunkt verschiebt. Zur Modellierung der Funktionsform von „reference-dependent preferences“ und deren Verhalten bei einer Verschiebung des Referenzpunktes vgl. z.B. Munro/Sugden (2003).
