Zusammenfassung
Das Jahr 2012 ist in Bezug auf die erblichen Typ-IV-Kollagenerkrankungen Alport-Syndrom und Syndrom der dünnen Basalmembran sehr ereignisreich: Zum einen schaffen drei retrospektive Analysen erstmals die Datenlage, dass die Erkrankungen ab dem Kindesalter mit ACE-Hemmern behandelbar sind und dass diese Behandlung die Lebenserwartung der Patienten verbessert. In der Folge erscheinen erstmals überhaupt offizielle Empfehlungen zur Diagnostik und Therapie beider Krankheiten. Und schließlich rekrutiert die weltweit erste Phase-III-Therapiestudie EARLY PRO-TECT Alport seit Sommer 2012 deutschlandweit Patienten. Zudem kooperieren NIH-gefördert China, Frankreich, USA, Kanada und Deutschland, um internationale Studien zu ermöglichen. Der Artikel fasst die jüngsten Entwicklungen praxisbezogen zusammen: Betont wird die essenzielle Funktion des Kindernephrologen bei der frühzeitigen Differenzialdiagnose der renalen Hämaturie und für den präemptiven Therapiebeginn. Betont wird auch die wichtige Rolle des Erwachsenennephrologen bei der Weiterbegleitung der jungen Erwachsenen und bei der prognoseorientierten Beratung von Patienten mit renaler Hämaturie. Jeder Nephrologe sollte seine Alport-Patienten unterweisen, dass Verwandte und Kinder mit familiären Hämaturieformen nun präventiv behandelt werden können.
Abstract
The year 2012 has been extremely eventful in relation to the hereditary type IV collagen diseases Alport syndrome and thin basement membrane disease: Three retrospective analyses provided the first data that these diseases can be treated with ACE inhibitors starting in early childhood and that therapy improves the life-expectancy of affected patients. As a result for the first time official recommendations for diagnosis and therapy of both diseases have been published. The world’s first phase III therapeutic clinical trial EARLY PRO-TECT Alport started recruitment throughout Germany in the summer of 2012. In addition, a NIH-funded cooperation between China, France, USA, Canada and Germany will allow international studies. The article summarizes recent developments for everyday clinical practice and emphasizes the essential role of pediatric nephrologists in the early differential diagnosis of renal hematuria and for pre-emptive therapy. It also stresses the important role of adult nephrologists in further support of young adults with Alport syndrome and in the prognosis-oriented consultation for patients with renal hematuria. Every nephrologist should instruct Alport patients that children and other relatives with familial hematuria or Alport syndrome now can be treated pre-emptively.
