Zusammenfassung
Hintergrund
Solange Typ-1-Diabetes-Patienten nicht deutschlandweit über alle Altersklassen hinweg in einem nationalen Diabetesregister erfasst werden, können die Häufigkeit und die Neuerkrankungsrate nur durch Zusammentragen der Forschungsergebnisse auf Basis der zur Verfügung stehenden, unterschiedlichen Datenquellen abgeschätzt werden.
Ziele der Arbeit
Dieser Beitrag liefert eine Zusammenstellung der verfügbaren Daten zur Inzidenz und Prävalenz des Typ-1-Diabetes im Kindes‑, Jugend- und Erwachsenenalter in Deutschland.
Material und Methoden
Mittels selektiver Literaturrecherche wurden aktuelle Studienergebnisse gesucht. Relevante Resultate wurden zusammenfassend aufbereitet und gegenüberstellend diskutiert.
Ergebnisse
Insgesamt deuten die vorliegenden Daten darauf hin, dass die Inzidenzrate des Typ-1-Diabetes im Kindes- und Jugendalter in den vergangenen Jahrzehnten zunahm. Der Inzidenztrend war in Deutschland regional verschieden, variierte zwischen Zeiträumen und war je nach Altersgruppe und Geschlecht unterschiedlich stark ausgeprägt. Nach einer aktuellen Hochrechnung beträgt die Inzidenz bei Kindern und Jugendlichen (≤17 Jahre) etwa 24 und bei Erwachsenen etwa 6/100.000 Personenjahre. Hierzulande erkranken jährlich etwa 3100 Kinder und Jugendliche sowie etwa 4100 Erwachsene an Typ-1-Diabetes. In Deutschland leben schätzungsweise insgesamt 32.000 Kinder und Jugendliche sowie etwa 335.000 Erwachsene mit Typ-1-Diabetes.
Diskussion
Die teilweise deutlich voneinander abweichenden Schätzungen der Inzidenz und Prävalenz sowie des Inzidenztrends aus den verschiedenen Studien sind vermutlich nicht nur auf Unterschiede in Methodik und genutzter Datenquelle, wie beispielsweise Register- und Sekundärdaten, zurückzuführen, sondern auch auf eine regionale Heterogenität der Krankheit.
Abstract
Background
As long as type 1 diabetes patients are not recorded nationwide across all age groups in a national diabetes registry, estimates of the incidence and prevalence of the disease can only be obtained by integration of research results based on various data sources that are available.
Objectives
This article provides a compilation of available data on the incidence and prevalence of type 1 diabetes in childhood, adolescence, and adulthood in Germany.
Materials and methods
A selective literature search was performed to identify recent study results. Relevant results were summarized and discussed.
Results
Overall, the available data suggest that the incidence rate of type 1 diabetes in childhood and adolescence has increased in past decades. The incidence trend was regionally different in Germany, varying between periods and varying in severity depending on age group and sex. According to current projections, the incidence in children and adolescents (≤17 years) is about 24 and in adults about 6 per 100,000 person–years. In Germany, about 3100 children and adolescents as well as about 4100 adults develop type 1 diabetes each year. It has been estimated that a total of 32,000 children and adolescents and about 335,000 adults with type 1 diabetes are living in Germany.
Discussion
The in part highly divergent estimates of incidence and prevalence as well as the incidence trend from various studies are probably not only attributable to differences in methodology and data sources used, such as registry and secondary data, but also indicate the regional heterogeneity of the disease.
