Zusammenfassung
Die Studie „Diabetes in Germany“, kurz DIG, untersucht prospektiv über 4 Jahre die Versorgungssituation von Typ-2-Diabetes-Patienten in Deutschland. Besondere Aufmerksamkeit gilt der Epidemiologie des metabolischen Syndroms und der atherosklerotischen Gefäßerkrankungen. Mehr als 4000 Patienten aus über 200 Arztpraxen nahmen an der DIG-Studie teil. Bei 74,4% der Patienten lag ein metabolisches Syndrom vor, 91,3% der Patienten hatten eine Hypertonie, und 49,8% waren adipös. Nur bei einer Minderheit lag die Zuckerkrankheit isoliert vor. In Bezug auf Folgeerkrankungen fielen Geschlechtsunterschiede auf: Frauen sind besonders gefährdet, wenn bei ihnen die Kombination aus Diabetes, Hypertonie, niedrigem HDL und Übergewicht vorliegt. Insgesamt kann man eine gute Blutzuckereinstellung von im Mittel 7,0% im HbA1c-Wert feststellen. Es bestehen Defizite bei der Behandlung von Hypertonie und Fettstoffwechselstörungen. Patienten mit Dyslipidämie werden zu 54% gar nicht und nur zu 17% leitliniengerecht therapiert. Die Versorgung mit Thrombozytenaggregationshemmern ist unzureichend.
Abstract
The “Diabetes in Germany” study (DIG) is a prospective study that has examined the healthcare situation of type-2 diabetes patients in Germany over 4 years. Special attention was paid to the epidemiology of the metabolic syndrome and the atherosclerotic vascular disease. More than 4,000 patients from over 200 doctors’ practices took part in DIG. In 74.4% of the patients a metabolic syndrome was present, while 91.3% had hypertension and 49.8% were overweight. Only a minority of patients had diabetes as a sole illness. With respect to late complications sex differences stand out: women are particularly at risk when they have a combination of diabetes, hypertension, low HDL and overweight. Overall, good blood glucose control was observed, with an average of 7.0% for HbA1c. There are deficits in the treatment of hypertension and lipid disorders. In 54% of patients with dyslipidaemia no therapy at all was given, and only 17% of patients were treated according to the guidelines. Thrombocyte aggregation inhibitors were not prescribed sufficiently often.
