Zusammenfassung
Husten ist ein Symptom beinahe aller pneumologischer und mehrerer extrapulmonaler Erkrankungen. Husten ist eine sehr häufige Beschwerde, da die banalen Infekte der oberen und unteren Atemwege („Erkältung“) akuten Husten verursachen, der in der Regel 3, in einigen Fällen bis zu 8 Wochen anhalten kann. Falls der Husten mehr als 8 Wochen anhält, sprechen wir von einem chronischen Husten. Der akute Husten muss nur ätiologisch abgeklärt werden, falls er mit „Alarmsymptomen“ wie Hämoptoe, hohem Fieber oder Atemnot vergesellschaftet ist. Der chronische Husten bedarf hingegen in jedem Fall einer sorgfältigen Abklärung, da er das Hauptsymptom einer bedrohlichen Erkrankung wie Tuberkulose oder Lungentumor sein könnte. Die Anamnese, eine körperliche Untersuchung, eine Röntgenaufnahme der Thoraxorgane und eine Lungenfunktionsprüfung helfen oft, die Ursache zu klären. In unklar gebliebenen Fällen könnten Asthma als Hustenäquivalent (sog. „cough variant asthma“), Erkrankungen der oberen Atemwege oder ein gastroösophagealer Reflux für den Husten verantwortlich sein.
Abstract
Cough is a symptom of almost all respiratory and some nonrespiratory diseases. Cough as a main complaint due to common colds is very frequent and usually lasts 3 weeks, in some cases however up to 8 weeks. As opposed to acute cough, where history and physical exam are sufficient, a chest X-ray and lung function test should be performed at presentation in the case of chronic cough lasting already 8 weeks and more. Chronic cough may be the only presenting symptom of dangerous diseases, i.e., tuberculosis or lung tumor. The chest X-ray and a lung function test help in most cases to establish the diagnosis. If they remain unremarkable and cough is the only presenting symptom an upper airway cough syndrome, cough variant asthma, or gastroesophageal reflux disease may be responsible.
