Die Versorgung epiphrenischer Ösophagusdivertikel in laparoskopisch-transhiataler Technik

Abstract

Introduction. The operative treatment for epiphenic diverticula is usually performed via thoracotomy or laparotomy.

Patients and methods. Alternatively we describe the feasibility of a laparoscopic transhiatal approach in three consecutive symptomatic patients with four epiphrenic diverticula. In all cases, the diverticula were completely resected laparoscopically in combination with an esophagomyotomy and a posterior partial fundoplication.

Results. The three patients showed uneventful postoperative courses with a speedy recovery and are symptom-free after a follow-up of 22–41 months.

Conclusion. Our results demonstrate that laparoscopic transhiatal procedure in patients with symptomatic epiphrenic diverticula can be performed safely, effectively, and gently.

Zusammenfassung

Hintergrund. Das operative Vorgehen bei epiphrenischen Divertikeln erfolgt üblicherweise über eine Thorakotomie oder Laparotomie.

Patienten und Methoden. Wir stellen an 3 Patienten mit 4 symptomatischen epiphrenischen Divertikeln die laparoskopisch-transhiatale Versorgung vor. In allen Fällen erfolgte laparoskopisch die vollständige Abtragung der Divertikel in Kombination mit einer Ösophagomyotomie und einer partiellen posterioren Fundoplicatio.

Ergebnisse. Die 3 Patienten zeigten bei unauffälligen postoperativen Verläufen eine schnelle Rekonvaleszenz und sind nach einer Beobachtungszeit von 22–41 Monaten völlig beschwerdefrei.

Schlussfolgerung. Die Ergebnisse zeigen, dass bei Patienten mit symptomatischen epiphrenischen Divertikeln eine operative Versorgung in laparoskopisch-transhiataler Technik sicher, erfolgreich und schonend möglich ist.
