Summary
High prevalence of chemical restraints in nursing homes is being criticised as suboptimal care. According to a new legal act, the Austrian "Heimaufenthaltsgesetz" 2005, nursing homes are obliged to report not only physical restraints but also chemical restraints. Using the Beers criteria we analysed all 78 notifications of chemical restraints registered during 18 months. Pronounced deficits concerning the selection of reporting criteria and the quality of doctors' documentation of prescription were observed. National guidelines defining chemical restraints and providing valid assessment instruments are urgently warranted.
Zusammenfassung
Die hohe Prävalenz von Medikamenten mit potentiell freiheitsbeschränkender Wirkung in Pflegeheimen wird international als suboptimale Versorgung kritisiert. Entsprechend dem österreichischen "Heimaufenthaltsgesetz" sind Pflegeheime nunmehr zur Meldung von mechanischen und auch medikamentösen Freiheitsbeschränkungen ihrer Bewohner verpflichtet. In 18 Monaten wurden insgesamt 78 Meldungen über medikamentöse Freiheitsbeschränkungen aus 23 Vorarlberger Pflegeheimen getätigt, die in dem vorliegenden Beitrag anhand der Beers-Kriterien analysiert werden. Es zeigen sich sowohl Defizite bei der Auswahl der Erhebungskriterien durch die Pflegeheime als auch bei der Medikamentendokumentation durch die verschreibenden Ärzte. Verbindliche Leitlinien zur Definition von medikamentöser Freiheitsbeschränkung und valide Instrumente für deren Erfassung und Beurteilung werden dringend benötigt.
