Zusammenfassung
Wir berichten über ein 5‑jähriges Mädchen, das im Rahmen eines Verkehrsunfalls ein Polytrauma erlitt. Nach der präklinischen Versorgung erfolgte im Schockraum bei zunehmend kreislaufinstabilem Kind und einseitig abgeschwächtem Atemgeräusch zunächst bei Verdacht einer endobronchialen Tubuslage eine Lagekorrektur des Tubus. Bei persistierend aufgehobenem Atemgeräusch links wurde bei vermutetem Spannungspneumothorax der Versuch einer linksseitigen Nadeldekompression mit einer 14Gauge(G)-Venenverweilkanüle in Monaldi-Position durchgeführt. Beim Zurückziehen des Mandrins zeigte sich ein pulsierender Blutfluss über die Kanüle. In der durchgeführten Bildgebung kamen eine Fehllage des Katheters in der Arteria (A.) pulmonalis sowie eine persistierende rechts-endobronchiale Lage des Tubus zur Darstellung. Nach Lagekorrektur des Tubus erfolgten eine operative Bergung des Katheters und eine Übernähung der Eintrittsstelle in die A. pulmonalis über eine Thorakotomie. Der Fall schildert die große, bei polytraumatisierten Kindern aus einer einseitigen Intubation resultierende, Gefahr eines fälschlicherweise vermuteten Spannungspneumothorax und die mit einer Nadeldekompression im Kindesalter assoziierten Komplikationsmöglichkeiten.
Abstract
This article reports the case of a 5-year-old girl who suffered multiple severe injuries in a road traffic accident. After preclinical treatment the child arrived at the emergency room in a critical condition with hypotension, tachycardia and unilateral severely compromised breathing. The uncuffed endotracheal tube was changed to a cuffed tube due to a massive air leakage hindering effective ventilation and oxygenation. Subsequently, absent breath sounds were noted in the left hemithorax that persisted after repositioning the endotracheal tube. With the suspicion of a tension pneumothorax a needle decompression of the chest was performed with a 14 gauge intravenous catheter in the Monaldi position. During withdrawal of the needle a pulsating blood flow over the catheter was observed. Thoracic computed tomography (CT) showed a malposition of the catheter in the pulmonary artery and a persisting right endobronchial position of the airway tube. After correction of the position of the tube surgical removal of the catheter and suturing of the entry point in the pulmonary artery were carried out via a thoracotomy. This case report illustrates the great danger of a falsely assumed tension pneumothorax in polytraumatized children resulting from a unilateral endobronchial intubation. Furthermore, it shows the high risk of complications associated with needle decompression in children.
