Zur Epidemiologie des plötzlichen Herztodes

Zusammenfassung

Der plötzliche Herztod (PHT) ist ein natürlicher und unerwarteter Tod aus kardialer Ursache, der innerhalb von 1 h nach Symptombeginn eintritt und durch eine plötzliche Bewusstlosigkeit angekündigt wird. Die Inzidenz des PHT wird auf 0,1–0,2%/Jahr der Gesamtbevölkerung geschätzt. Sie steigt jenseits des 60. Lebensjahres deutlich an. Der Anteil des plötzlichen Herztodes an allen Todesfällen macht etwa 10–20% aus. Auslöser ist meist eine tachykarde Herzrhythmusstörung. Bedroht sind vor allem Personen mit akutem Myokardinfarkt. Weiterhin sind Patienten mit stark eingeschränkter linksventrikulärer Pumpfunktion auf dem Boden einer chronischen, strukturellen Herzerkrankung gefährdet. Weitere Indikatoren für einen drohenden PHT können durch nichtinvasive, elektrophysiologische Analysen aufgedeckt werden.

Abstract

Sudden cardiac death (SCD) is a natural and unexpected death of cardiac origin that occurs within 1 h after onset of symptoms and is heralded by sudden unconsciousness. The incidence of SCD is estimated at 0.1–0.2% of the total population. It increases noticeably after the age of 60. SCD accounts for approximately 10–20% of all fatalities. Cardiac arrhythmia is usually the triggering factor. Persons suffering acute myocardial infarction are especially at risk. Patients with severely limited left ventricular pump function due to chronic structural heart disease remain at risk. Further indicators of imminent SDC can be revealed by noninvasive electrophysiological analyses.
