Der Druckplattenfixateur interne der HWS als winkelstabiles Implantat wird seit Jahren in der Behandlung von instabilen HWS-Verletzungen erfolgreich eingesetzt. Die Erforschung des Prinzips der Winkelstabilität führte zur Entwicklung eines neuen Implantats. Dabei wird ein härterer, gewindetragender Schraubenkopf in ein weiches Titanplattenloch eingedrängt. Durch den Verzicht auf die Druckplatte des Druckplattenfixateurs kann der Titanfixateur interne kleiner dimensioniert werden, die Schraubenrichtung ist frei wählbar, und in Bezug auf Rotation und Biegung wird eine höhere Stabilität erreicht. Die Analyse der ersten 13 im Rahmen einer ventralen Spondylodese mit dem neuen Implantat versorgten Patienten zeigt, daß bei guter Funktion der HWS und regelrechtem Heilungsverlauf eine Implantatlockerung oder ein Stellungsverlust der Fusion nicht nachweisbar sind. Bei fraglicher Infektsituation, Spondylitiden und zu erwartender Tracheotomie mit Sekundärkontamination kommt aufgrund der Möglichkeit, Refobacin-haltigen Knochenzement unter der Druckplatte zu applizieren, der Druckplattenfixateur weiterhin zum Einsatz. Der neue Titanfixateur interne für die HWS findet bei einfacher Handhabung und hoher Winkelstabilität zunehmend bei der Standardversorgung frischer HWS-Verletzungen Anwendung.
Internal compression plate fixation (DPF) for treatment of unstable cervical spine injuries has been in clinical use for some years already. We report about a new angular stable interlocking plate developed at our institution. Study of the principle of angular stability led to the design of a new implant. The design of our internal titanium fixation system consists of a plate made of titanium in which self-drilling screws interlock, thereby providing angular stability between plate and screws and allowing for better stability during rotation and bending of the cervical spine. The angle of the screws can be selected by the surgeon and the size of the plate reduced by omitting the compression plate. So far, 13 patients with cervical trauma have been treated with a ventral spondylodesis using the new implant. Follow-ups show good functioning of the cervical spine. No implant-related complications or loss of correction have been observed. The DPF is still used in patients with inflammatory reactions, spondylitis, and secondary contamination such as tracheostomas, because it allows the application of bone cement containing antibiotics to the compression-plate.
