Die Behandlung von Pseudarthrosen, posttraumatischen Fehlstellungen und Refrakturen des Oberschenkels bedarf bei häufig schlechter lokaler Vaskularisation einer besonderen Berücksichtigung der Biologie und der Biomechanik. Auch bei Frakturen sollte dies beachtet werden. Um dem Rechnung zu tragen, wurde ein winkelstabiles Implantat, der Druckplattenfixateur interne, für das Femur (DPF) entwickelt. Von Januar 1993 bis Dezember 1997 wurden bei 43 Patienten 45 DPF implantiert (19 Pseudarthrosen, 14 frische Frakturen, 6 Refrakturen und 5 posttraumatische Fehlstellungen). 41 Patienten wurden nach durchschnittlich 2 Jahren nachuntersucht und über 1 Patienten telefonisch Auskunft eingeholt. In allen Fällen kam es zu einer knöchernen Durchbauung, wobei nur bei 2 Patienten Sekundäreingriffe notwendig waren. 1mal kam es bei einer Pseudarthrose zu einem Plattenbruch (nach erneuter DPF-Anlage jetzt durchbaut), 1 Fehlstellung wurde nicht komplett korrigiert und 1mal kam es zu Thrombosen. Aus unseren Ergebnissen schließen wir, daß der DPF zur Behandlung von Pseudarthrosen, Refrakturen, posttraumatischen Fehlstellungen und Frakturen ein gut geeignetes Implantat darstellt.
In the treatment of pseudarthrosis, post-traumatic axis deviations , or refractures of the femur, special consideration must be taken with biology and biomechanics, because local vascularization is often damaged. The same applies to fractures. To this purpose, we developed a special plate with angular stability, the “Druckplattenfixateur” (DPF), or compression plate fixation system, for the femur. From January 1993 to December 1997, 45 DPFs were implanted in 43 patients (19 cases of pseudarthrosis, 14 fractures, six refractures, and five post-traumatic deviations of the axis). Forty-one patients were examined an average of 2 years postoperatively and the relatives of one were interviewed by telephone. In every case, bony healing occurred, and in only two cases were secondary operations necessary. In one pseudarthrosis case, we saw breakage of the plate and, after revision with a new DPF, bony healing. In one case, a deviation of the axis was not completely corrected and in another there was a deep vein thrombosis. Our results have led us to the conclusion that the DPF is a suitable implant for the treatment of pseudarthrosis, fractures, refractures, and post-traumatic axis deviations of the femur.
