Zusammenfassung
Das perioperative Gerinnungsmanagement in der muskuloskeletalen Chirurgie stellt zahlreiche Herausforderungen an Chirurgen und Anästhesisten. Perioperativ muss eine Balance zwischen dem Risiko für Blutungskomplikationen und dem thromboembolischen Risiko gefunden werden. Der Umgang mit gerinnungshemmenden Medikamenten ist hierbei nicht immer einfach. Es muss entschieden werden, ob eine bestehende Antikoagulation oder antithrombozytäre Therapie pausiert werden kann, zu welchem Zeitpunkt dies geschehen muss und ob eine überbrückende Therapie erforderlich ist. Neben den verschiedenen Gruppen gerinnungshemmender Medikamente wie Thrombozytenaggregationshemmern, Vitamin-K-Antagonisten und den direkten oralen Antikoagulanzien (DOAK) müssen individuelle Risikofaktoren der Grunderkrankung und das eingriffsassoziierte Blutungsrisiko beachtet werden. Im Notfall stehen verschiedene Strategien zur Verbesserung der Gerinnungssituation zur Verfügung, da bei Blutungskomplikationen die Gerinnungssituation schnell verbessert werden muss. In diesem Beitrag werden die aktuellen Empfehlungen zum perioperativen Umgang mit gerinnungshemmenden Medikamenten und Strategien zur Abwägung zwischen Blutungs- und Thromboembolierisiko dargestellt. Außerdem wird beschrieben, wie man bei perioperativen Blutungen unter Therapie mit gerinnungshemmenden Medikamenten reagieren sollte.
Abstract
In terms of perioperative coagulation management in musculoskeletal surgery, surgeons and anesthesiologists face various challenges. Perioperatively, a balance between the risk of bleeding complications and thromboembolic risks must be found. Dealing with anticoagulant drugs is not always easy. It has to be decided whether treatment with anticoagulants or platelet aggregation inhibitors can be paused, at what time this should happen and if a bridging treatment is necessary. In addition to the several groups of anticoagulant drugs, such as platelet aggregation inhibitors, vitamin K antagonists and direct oral anticoagulants (DOAC), individual risk factors and the intervention-associated risk of bleeding must be considered. In an emergency various strategies are available to improve the coagulation situation, because in the case of bleeding complications the coagulation situation has to be quickly improved. This article presents the current recommendations on perioperative management of anticoagulant drugs and strategies to weigh up the bleeding risk and the risk of thromboembolism. A description of how to respond to perioperative bleeding under treatment with anticoagulants is also given.
